Medical Records & Forms
WebMedical Records & Forms. All incoming students are required to submit a medical record to Bucknell Student Health prior to arrival on campus. Medical records must be submitted using Bucknell's official record form. The form includes a medical provider packet that must be completed by a physician within twelve months of the start of classes.
