Resumption of Care Assessment
WEBThe Resumption of Care order contains the Plan of Care for the remainder of the existing certification period for the patient. For example, the patient is hospitalized over 24 hours for reasons other than testing in the middle of of the certification period. The resumption assessment is completed when the patient returns home.

Plan of Care/485 Overview
WEBThe 485 defaults to a status of open. For the open 485, you can view all current active services from the Care Plan for the certification on the order. Changes made to services on the Care plan flow through to the open 485. After the 485 is sent to the physician for signature, services freeze and are no longer updated from the Care Plan.
Selecting a Client
WEBFrom the Home screen, click Clients and do one of the following: To search using manual entry: Click the search field of the patient's Last Name or first name. Begin typing the sequential number in the medical record number Social Security Number (SSN). Highlight the patient name you want to select and click Accept.
Verifying Eligibility In Intakes
WEBTo edit an existing insurance, click Edit. Complete the fields as required. Click Accept. Eligibility Verification automatically processes. To re-verify eligibility, from Intakes. Click Edit next to the Patient name. Under Patient section, click Edit Patient . Under Insurance Coverage, Eligibility Status, click View. Click Verify Eligibility.

OASIS Correction Tools
WEBSteps. Within the clients chart, click Case Details. Click Oasis and then click Correct . Click Correct next to the OASIS record to be updated. A new OASIS record is generated in an incomplete status. Within the OASIS table, in the # column, you can see a 01 for the first correction. Make changes as needed and mark record Reviewed.
Continuity Of Care Document Overview
WEBThe Continuity of Care Document (CCD) is located within the client header of the patient chart in the Home Care Home Health Care. The CCD can be used to share information regarding patient details. For example, transfer to a hospital or outside provider. The CCD is created automatically from the content located in the patient record.

OASIS Correction Tools
WEBWithin the client chart, click Case Details and click OASIS. Click Inactivate next to the OASIS with the incorrect information. The page reloads with a copy of the original OASIS in a reviewed status and XX in the # column of the table, which indicates the OASIS is inactivated. Nothing more needs to be done with the OASIS assessment.
Managing Dashboards
